How to prepare for your consultation:
Please print and complete the form below and bring it with you to your appointment. If you do not have printer access, please review the form and we will have a copy available for you to complete when you arrive.
Please arrive 10 minutes prior to your appointment time to allow for form completion.
During the consultation we will review your medical history and then discuss your contraceptive needs.
How to prepare for your IUD or implant insertion appointment:
Taking an anti-inflammatory medication approximately one hour prior to your appointment can help with discomfort post insertion.
Remember to be able to pee when you arrive so that we can complete a urine pregnancy test prior to insertion.
Please arrive 10 minutes prior to you appointment to facilitate the above preparation.
